Objectives: Although renal insufficiency is associated with high mortality in \rpatients with acute coronary syndrome, studies have often excluded patients \rof advanced age who have renal insufficiency. The present study aimed to \rdetermine the relationship between renal insufficiency and mortality predictors \rin patients with non-ST-segment elevation myocardial infarction (NSTEMI) who \rwere aged 65 and older.\rMethods: The study included 537 NSTEMI patients aged 65 years and \rover, who were followed up for at least 12 months, who were admitted to the \rhospital with the diagnosis of NSTEMI and underwent coronary angiography. \rSociodemographic and clinical features, laboratory parameters, and clinical \revaluation variables were recorded at the time of admission. The patients \rwere divided into four groups according to their ages and creatinine clearance \rvalues. \rResults: The mean age of the patients was 76.17 ± 6.64 years, and 256 (47.7%) \rof them were women. According to the applied multivariate age-stratified Cox \rregression analysis, independent predictors of one-year mortality were found \rto be creatinine clearance (p=0.005), left ventricular ejection fraction (p<0.001), \rand coronary revascularization (p=0.004). \rConclusion: Creatinine clearance, left ventricular ejection fraction, and \rcoronary revascularization are robust and independent predictors of one-year \rmortality in NSTEMI patients.
